Get Started As a Referral Partner

To start using Activate Care's Referrals module, you must be first invited by your local CareHub.  Look out for an email invitation from help@activatecare.com or support@activatecare.com

Create Your Account

The first step of joining your local CareHub as a Referrals Partner is finding your invitation email.  Once you have found your invitation email click Set up your account

The link should bring you to our Enrollment page where you need to create your password (Your username is your email address).

Your password must have:

  • 1 uppercase letter
  • 1 lowercase letter
  • 1 number
  • 1 special character

NOTE: You will be asked to certify that you have read, understood and agree to the Activate Care Terms of Service and Privacy Policy as well as the Hub Agreement (relationship between CareHub and Participating Organizations)

Click Create to complete account creation process.

Log In to Activate Care

After creating your account, log in here with your email and newly created password

  1. Confirm/complete their organization information 
    1. Required: Name & Description
    2. Optional: location, phone numbers, email, operating hours 
  2. Confirm/complete their program(s) information
    1. Required: Name Name, Description, Needs this program serves 
      1. Only one need required
    2. Recommended: contact info, eligibility criteria, geographies covered, cost, operating hours 
  3. Turn on “Accepting Referrals”

  4. User management 
    1. Add/invite staff as needed 
    2. Training materials available on the help desk
